Job description

Dynamic is looking for professional, reliable, and caring Full-time or Per Diem Home Health Licensed Vocational Nurses (LVNs) to join our team! We are family-owned and operated, specializing in at-home rehabilitation post-hospitalization. Our agency is Centers for Medicare and Medicaid Services (CMS) certified, and we have been providing excellent in-home care to patients across the Greater LA and Ventura County communities for over 37 years.

To comply with CMS regulations, Candidates must have at least one year of experience as an LVN in an acute care facility or other care settings, and proficiency in wound care is a huge plus! Our Home Health Care professionals must have integrity & dedication to patient care, as well as EXCELLENT interpersonal communication skills. Previous Home Health Experience is another huge plus!

Responsibilities:

  • Administer nursing care to ill, injured, or disabled patients
  • Diagnose and establish patient treatment plans
  • Monitor and report changes in patient symptoms or behavior
  • Communicate with collaborating physicians or specialists regarding patient care
  • Educate patients about health maintenance and disease prevention
  • Maintain accurate patient medical records
  • Provide advice and emotional support to patients and their family members
  • Being bilingual in Spanish is a huge plus!

Qualifications:

  • At least 1 year of clinical experience as a licensed LVN
  • Familiarity with medical software (EMR) and Medical equipment
  • Ability to build rapport with patients
  • Strong problem-solving and critical thinking skills
  • Ability to thrive in a fast-paced environment
