Part - Time Licensed Vocational Nurse (LVN)
Job Number: 26-12341
Job Summary: This Licensed Vocational Nurse (LVN) position will work to serve clients in the new crisis residential Therapeutic Diversion Program (TDP) at 15th St. The primary duties is to provide general health care support, training and education to clients in a clinical environment for ongoing stabilization with therapeutic interventions, relapse prevention services and facilitating transition and linkage for continuity of care to individuals experiencing challenges with mental health and/or substance abuse, homelessness and criminal justice involvement within the community. The LVN will assist in guiding and coordinating clients' treatment planning, including their desired goals and identified tasks for client self-care, with applicable education, training, referrals, and resources. The LVN will work alongside the multidisciplinary clinical team to comply with contractual criteria, agency guidelines, and program requirements. The LVN will work in close collaboration with the TDP RN Supervisor (RNS) & the Practice Manager (PM), as well as the TDP staff and other collaborating partners, to continuously assess, develop, and refine the service delivery system to assist clients' success, offering optimal care with compassion, trauma-informed practices that promote racial and health equity, and using evidence-based practices. TDP is designed to offer longer-term stabilization based on the clients' needs and goals while residing at the 15th St. unit. The LVN, along with the RNS, in conjunction with the PM, will engage with other community supporting stakeholders to promote cohesive, comprehensive, quality and effective therapeutic diversion and deflection for individuals to be able to establish a productive and positive foundation for sustainable, independent, and healthier living.
This position is under the administrative supervision and clinical oversight of the program's RNS.
Regular, predictable attendance is considered to be an essential requirement for this position. Other duties may be assigned. May require a flexible work schedule.

Required:
  • Graduation from an accredited LVN program.
License:
Required:
  • Current LVN (Licensed Vocational Nurse) license.
  • Current and valid driver's license and a clean driving record.
Experience:
Preferred:
  • 1 year of direct care experience.
  • Bilingual in Spanish.
Hours: Part - Time.
Workdays: Saturdays and Sundays.
Workhours: 8:00AM - 8:00PM.
Salary: $29.39 - $37.47 an hour, depending on years of licensure, plus an additional 10% crisis stipend.
Position may be eligible for an additional shift differential pay.
  • Second Shift: $3.25/hour.
  • Third Shift: $7.25/hour.
  • Weekend Shift: $8.00/hour.
