LVN - HOME HEALTH

Position OverviewPosition Summary

As a Licensed Vocational Nurse (LVN) – Home Health, you will provide skilled nursing care and related services to patients in their homes under the direction of a Registered Nurse (RN) or physician. You will play a vital role in implementing individualized care plans, educating patients and families, administering treatments, and supporting positive patient outcomes.

This position may include participation in an on-call rotation based on agency needs and location requirements.

Responsibilities
  • Deliver skilled nursing care under the supervision of an RN

  • Conduct routine patient home visits according to agency policies

  • Assist in developing and implementing individualized care plans

  • Monitor patient conditions and communicate changes to the care team

  • Administer medications and prescribed treatments

  • Perform clinical procedures including:

    • Injections

    • Phlebotomy

    • Drug screenings

    • Wound care and sterile dressing changes

  • Educate patients and families regarding treatment plans and self-care

  • Support discharge planning and patient goal achievement

  • Coordinate care with interdisciplinary team members

  • Maintain accurate clinical documentation

  • Order and restock medical supplies as needed

QualificationsRequired
  • Current Licensed Vocational Nurse (LVN) license in the state of practice

  • Current BLS (Basic Life Support) certification from an approved provider

  • Strong clinical assessment and patient care skills

Preferred
  • Bachelor's Degree

  • Arrhythmia certification

  • Valid driver's license

  • Previous home health experience
