Job description

Deliver skilled care for patients with complex, long‐term medical needs.

We’re seeking experienced and compassionate LTAC LVNs to support patients requiring extended hospital‐level care. In this role, you’ll work closely with RNs and interdisciplinary teams to provide consistent, high‐quality care for medically complex patients over longer lengths of stay.

Schedule

  • 7p | 12‐Hour Shifts
  • Per Diem flexibility

What You’ll Do

  • Provide direct patient care under the supervision of a Registered Nurse
  • Monitor vital signs, neurological status, and overall patient condition
  • Administer medications and treatments per provider and RN orders
  • Assist with care of patients with tracheostomies, feeding tubes, complex wounds, and central lines (within scope)
  • Support ventilator‐dependent and chronically ill patients in collaboration with RNs and respiratory therapists
  • Assist patients with activities of daily living (ADLs), mobility, and positioning
  • Observe, report, and document changes in patient condition promptly
  • Collaborate with interdisciplinary care teams to support patient stability and progression
  • Document care activities, assessments, and patient responses accurately in the medical record

What We’re Looking For

  • Active LVN license in the state of practice
  • Minimum of 1 year of recent LTAC, acute care (hospital), step‐down, or ICU‐adjacent LVN experience required
  • BLS certification required
  • IV Certification required
  • Experience caring for medically complex or chronically ill patients
  • Strong clinical observation, communication, and documentation skills
  • Ability to work effectively in a fast‐paced, high‐acuity environment
  • Team‐oriented mindset with a strong commitment to patient‐centered care
