Job Summary
We are seeking a compassionate and detail-oriented Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will provide high-quality patient care across various settings, including long-term care, assisted living, hospitals, and outpatient clinics. This role involves performing comprehensive assessments, administering medications and treatments, and collaborating with multidisciplinary teams to ensure optimal patient outcomes. Experience with EMR/EHR systems such as Epic, Cerner, or Athenahealth is preferred. The LPN will play a vital role in supporting patients with diverse needs, including geriatrics, memory care, acute pain management, and infectious disease care. A strong understanding of medical terminology, anatomy, physiology, and infection control protocols is essential for success in this position.

Duties

  • Conduct patient assessments including vital signs, physical examinations, and intake evaluations
  • Administer medications and injections accurately following physician orders
  • Perform IV infusions, catheterizations, phlebotomy, and sterile procedures
  • Assist with diagnostic evaluations such as laboratory specimen collection and processing
  • Manage airway care, ventilator support, and tube feeding as needed
  • Document patient care activities thoroughly in EMR/EHR systems like Epic or Cerner
  • Provide wound suturing and dressing changes while maintaining aseptic technique
  • Support discharge planning and patient education for post-acute or outpatient care
  • Collaborate with physicians and healthcare teams in acute care settings such as ICU or trauma centers

Requirements

  • Valid Licensed Practical Nurse (LPN) license in the state of practice
  • Proven experience in diverse clinical environments such as hospital medicine, emergency room, ICU, or long-term care facilities
  • Proficiency with EMR/EHR systems including Epic, Cerner, or Athenahealth preferred
