Licensed Practical Nurse (LPN)
Position Summary:
The Licensed Practical Nurse (LPN) provides direct nursing care to residents and supports the implementation of care plans under the supervision of a Registered Nurse (RN). The LPN plays a key role in promoting resident health, safety, and well-being through skilled clinical care, communication, and teamwork.
Key Responsibilities:
  • Provide direct nursing care to residents in accordance with care plans and physician orders
  • Administer medications and treatments accurately and safely
  • Monitor residents' health status, recognize changes, and report promptly to the RN or physician
  • Document assessments, interventions, and resident responses in compliance with regulations
  • Supervise and support Nursing Assistants (CNAs) and Trained Medication Aides (TMAs)
  • Collaborate with the interdisciplinary team to promote resident-centered care
  • Ensure adherence to infection control, safety, and regulatory guidelines
  • Promote residents' dignity, independence, and comfort at all times
Qualifications:
  • Current and active LPN license in Minnesota
  • Knowledge of nursing principles, practices, and state/federal regulations
