About the Role

Join myPlace Health as a Licensed Practical Nurse (LPN) in Los Angeles, CA. myPlace Health is dedicated to enabling older adults to live independent lives by providing value-based, comprehensive care and coverage, allowing them to thrive in their homes and communities. As an LPN, you will play a crucial role in our high-touch, team-based medical care, ensuring our participants receive personalized and integrated support.

Key Responsibilities
  • Provide direct nursing care under the supervision of a registered nurse or physician.
  • Administer medications and treatments as prescribed.
  • Monitor and document participant vital signs and health status.
  • Assist with participant assessments and care plan development.
  • Communicate effectively with participants, families, and the interdisciplinary team at myPlace Health.
  • Educate participants and caregivers on health maintenance and disease prevention.
  • Maintain accurate and detailed medical records.
  • Respond to emergent situations in a calm and efficient manner.
Required Qualifications & Experience

Required:

  • Minimum 2 years of experience as a Licensed Practical/Vocational Nurse.
  • Ability to work overtime and night shifts.
  • Current LPN/LVN license in the state of California.
