Licensed Practical Nurse (LPN) – Weekend Double Shift (1st & 2nd Shift)About Us

Job Summary

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our interdisciplinary team. This position is scheduled to work weekend double shifts covering 1st and 2nd shifts, providing direct nursing care and support to residents in our long-term care and rehabilitation community.

The ideal candidate will be committed to delivering quality resident care, maintaining regulatory compliance, and collaborating with physicians, registered nurses, therapists, and other healthcare professionals to promote positive resident outcomes.

Responsibilities
  • Administer medications and treatments as prescribed by physicians.
  • Monitor residents' conditions and report changes to the supervising nurse or physician.
  • Accurately document assessments, treatments, and resident care activities.
  • Assist residents with activities of daily living (ADLs) as needed.
  • Collaborate with the rehabilitation team to support resident recovery goals.
  • Provide education and support to residents and their families.
  • Maintain a safe, clean, and comfortable environment for residents.
  • Follow infection prevention, safety, and facility policies and procedures.
  • Ensure compliance with all state and federal regulations governing long-term care.
Qualifications
  • Current and unrestricted LPN license in the state of employment.
  • Previous long-term care or rehabilitation experience preferred, but not required.
  • Strong clinical and critical-thinking skills.
  • Excellent communication and interpersonal abilities.
  • Ability to work effectively in a team-oriented environment.
  • Compassionate, resident-focused approach to care.
  • Current BLS/CPR certification required.
