Licensed Practical Nurse (LPN) plays a vital role in our organization, and we are looking for a healthcare professional who shares our passion for delivering high-quality care.

Job Summary:

As a Licensed Practical Nurse (LPN) at Bayshore Nursing & Rehab, you will be responsible for providing direct patient care, administering medications, and performing various medical procedures. You will work closely with our interdisciplinary team to develop and implement individualized care plans, ensuring our patients receive the best possible outcome.

Responsibilities:

  • Provide direct patient care, including vital sign monitoring, wound care, and mobility assistance

  • Administer medications and treatments as prescribed by physicians

  • Perform various medical procedures, such as injections, venipuncture, and ECGs

  • Develop and implement individualized care plans in collaboration with the healthcare team

  • Monitor and report patient progress to the healthcare team

  • Maintain accurate and detailed records of patient care and treatment

  • Collaborate with other healthcare professionals to ensure seamless care coordination

  • Demonstrate exceptional communication and interpersonal skills to ensure patient satisfaction and family involvement

Requirements:

  • Current licensure as a Licensed Practical Nurse (LPN) in the state of Wisconsin

  • High school diploma or equivalent required

  • One year of experience in an acute care or long-term care setting preferred

  • Ability to work effectively in a fast-paced environment with frequent interruptions

  • Strong attention to detail and organizational skills

  • Ability to lift, push, and pull patients as needed
