Licensed Practical Nurse (LPN)
Job Description:
The Licensed Practical Nurse (LPN) is responsible for providing high-quality, compassionate nursing care to residents in accordance with physician orders, facility policies, and regulatory standards. The LPN assists in planning, implementing, and evaluating nursing care while ensuring the comfort, safety, and well-being of residents. This role works under the direction of the Director of Nursing and plays a vital role in promoting resident dignity, independence, and satisfaction.
Essential Duties & Responsibilities
  • Provides direct nursing care to residents, including administering medications, treatments, and wound care as prescribed.
  • Monitors residents for changes in condition and promptly reports concerns to the RN, physician, or appropriate healthcare provider.
  • Documents nursing observations, interventions, and resident responses accurately and in accordance with facility policies.
  • Assists with resident assessments and contributes to the development and implementation of individualized care plans.
  • Oversees and supports Certified Nursing Assistants (CNAs) to ensure residents receive appropriate care and assistance.
  • Maintains compliance with infection control, safety, and regulatory guidelines.
  • Participates in shift handoffs to ensure continuity of resident care.
  • Responds quickly and appropriately to medical emergencies and urgent resident needs.
  • Provides emotional support and education to residents and their families.
  • Participates in training, performance improvement, and quality assurance initiatives as directed.
  • Completes assignments and tasks assigned by the Director of Nursing as needed.

Job Knowledge & Skills:
  • Comprehensive knowledge of nursing principles, long-term care practices, and geriatric health needs.
  • Proficiency in medication administration, wound care, and documentation practices.
  • Understanding of infection control, safety, and resident rights regulations.
  • Ability to supervise, instruct, and support nursing assistants and other staff.
  • Strong communication, observation, and critical-thinking skills.
  • Ability to remain calm and make sound decisions in high-pressure or emergency situations.
  • Compassionate and professional approach to working with residents, families, and team members.

Qualifications & Education:
  • Current and active Licensed Practical Nurse (LPN) license in the state of Oklahoma.
  • Graduate of an accredited practical nursing program.
  • Minimum of 1 year of nursing experience in a long-term care, skilled nursing, or similar healthcare setting preferred.
  • Current CPR and First Aid certification (or ability to obtain upon hire).
  • Must demonstrate a moderate level of computer literacy in order to use and access our systems and tools within our facilities to complete online tasks and documentation.
  • Must be able to effectively communicate and interact professionally with staff and residents.

Physical Requirements:
  • Ability to stand, walk, and move throughout the facility for extended periods.
  • Must be able to lift, push, pull, and move up to 50 pounds.
  • Frequent bending, reaching, stooping, and assisting residents with mobility and care needs.
  • Visual and auditory acuity required for monitoring residents and equipment.
  • Physical and mental stamina to respond quickly to resident needs and emergencies.
