Requirements
- Current Nursing License in the practicing state
- Valid Driver's License
- Current CPR Certification
- Two years of experience as an LPN in a clinical setting, preferably in a home health or hospice setting
- Proficiency in electronic medical charting and time-keeping software
Roles & Responsibilities
- Administer skilled nursing care to patients in their residence as prescribed by a physician and in accordance with the established plan of care.
- Assist in identifying patients' physical, psycho-social, and environmental needs, and report outcomes to the supervising RN.
- Participate in planning, coordinating, and implementing care in conjunction with the RN to provide quality patient care.
- Provide effective communication to patients/families and co-workers, properly documenting outcomes of interventions.
- Assist patients and families in learning appropriate self-care techniques and assess their responses to care.
- Ensure care is provided utilizing infection control measures.
Compliance / Notes
- Proof of current Hepatitis consent/declination and TB vaccination record is required.
- Must read, write, and speak fluent English.
- Must have good and regular attendance.
- Additional duties may be assigned as needed.
