Office Location: 325 N Corporate Dr Suite 260, Brookfield, WI 53045
Coverage area:  Travel is required. Will cover Milwaukee and Waukesha Counties

Schedule: Full Time Monday-Friday 8AM-4:30PM

As a Hospice LPN You will: 

  • Give hands-on care to patients nearing the end of life, following state nursing rules.
  • Follow the care plan after assessing the patient’s needs.
  • Keep clear and timely records of the care provided.
  • Teach and support patients and their families about the illness and care plan.
  • Help coordinate care by communicating with the hospice team (like aides, chaplains, social workers).
  • Manage pain and symptoms, checking how well treatments are working.
  • Take part in training and quality improvement activities to grow professionally and improve care.

Qualifications

  • Graduate of an accredited practical nurse or vocational nursing program.
  • Three years nursing experience. Community health/hospice or medical/surgical experience is preferred.
  • Currently licensed as an LPN/LVN in the State.
  • Comply with accepted professional standards and practice.
  • Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.
  • Possess and maintain current CPR certification.
  • Understand philosophy of hospice concept. Also understand needs of the terminally ill.
  • Use own knowledge and experience base and other resources as necessary to make logical decisions and solve problems.
  • Demonstrate good verbal and written communication, and organization skills.
