We are a compassionate, client-centered home health care agency dedicated to delivering exceptional care in the comfort of our clients' homes. We’re currently seeking a Licensed Practical Nurse (LPN) to join our growing team for on-call and per diem home visits.
What You’ll Do:

  • Provide skilled nursing care to clients in their homes under the direction of a Registered Nurse.
  • Conduct timely and thorough assessments and deliver treatments based on physician orders and care plans.
  • Educate clients and families on health conditions and self-care.
  • Document all care activities in accordance with agency protocols and EMR requirements.
  • Respond to client needs during assigned on-call shifts and per diem requests with professionalism and empathy.
  • Collaborate with interdisciplinary teams to ensure continuity and quality of care.
Who You Are:
Licensed Practical Nurse (LPN) in the state of Connecticut
1+ year of nursing experience (home health preferred)
Bilingual (Spanish/English) highly preferred to support communication with our diverse patient population.
Strong communication, critical thinking, and documentation skills
Comfortable working independently in home settings
Reliable transportation, valid driver’s license, and auto insurance
Tech-savvy and familiar with EMR systems (training provided)
