What You’ll Do:

  • Provide skilled nursing care to clients in their homes under the direction of a Registered Nurse.
  • Conduct timely and thorough assessments and deliver treatments based on physician orders and care plans.
  • Educate clients and families on health conditions and self-care.
  • Document all care activities in accordance with agency protocols and EMR requirements.

Who You Are:
Licensed Practical Nurse (LPN) in the state of Connecticut
