Licensed Practical Nurse (LPN)

The Licensed Practical Nurse (LPN) provides high-quality nursing care under the direction of a physician, advanced practice provider, or Registered Nurse in accordance with the South Carolina Nurse Practice Act. The LPN is responsible for assessing patient needs, delivering direct patient care, assisting with examinations and procedures, administering medications and treatments, coordinating care, and educating patients and their families. This position plays a vital role in supporting the mission of Health Care Partners of South Carolina by delivering compassionate, patient-centered care while promoting positive health outcomes for the communities we serve.

Essential Duties and Responsibilities

· Prepare patients for examination by obtaining and documenting medical histories, chief complaints, vital signs, and other clinical information in the Electronic Health Record (EHR).

· Assess patient needs through observation, communication, physical assessment, telephone triage, and review of diagnostic information, reporting significant findings to the provider or Registered Nurse.

· Provide direct patient care, including assessments, treatments, medication administration, immunizations, injections, wound care, dressing changes, and other nursing procedures within the LPN scope of practice.

· Perform clinical procedures including specimen collection, electrocardiograms (EKGs), point-of-care testing, inhalation treatments, and other delegated duties as appropriate.

· Assist physicians and advanced practice providers with examinations, treatments, and minor procedures.

· Educate patients and families regarding medications, preventive care, chronic disease management, treatment plans, discharge instructions, and follow-up care.

· Coordinate laboratory testing, referrals, diagnostic procedures, and follow-up appointments as directed by the provider.

· Conduct telephone triage and follow-up calls, providing appropriate guidance within the scope of practice and escalating concerns to the provider when necessary.

· Accurately document patient assessments, treatments, communications, and clinical activities in the Electronic Health Record (EHR) in a timely manner.

· Support quality improvement initiatives by participating in chart audits, quality assurance activities, and organizational initiatives, including HRSA Uniform Data System (UDS) measures.

· Maintain examination rooms, medical equipment, and clinical supplies while ensuring compliance with OSHA, infection prevention, and safety standards.

· Monitor medical supplies, medications, and vaccines for inventory levels and expiration dates, and notify appropriate personnel of equipment or supply needs.

· Maintain patient confidentiality and protect health information in accordance with HIPAA and organizational policies.

· Respond appropriately to medical emergencies following established clinical protocols.

· Collaborate with providers, nursing staff, and interdisciplinary care teams to ensure coordinated, patient-centered care.

· Participate in required trainings, continuing education, staff meetings, and organizational quality improvement initiatives.

· Travel between Health Care Partners of South Carolina clinic locations as needed.

· Perform other duties as assigned.

Education & Experience

· Graduate of an accredited Practical Nursing program required.

· Current, unrestricted South Carolina Licensed Practical Nurse (LPN) license required.

· Current Basic Life Support (BLS) certification through the American Heart Association or American Red Cross required.

· Minimum of one (1) year of clinical nursing experience in a physician practice, outpatient clinic, community health center, or similar healthcare setting preferred.

· Experience working in a Federally Qualified Health Center (FQHC) or community health center preferred.

· Experience using Electronic Health Record (EHR) systems and Microsoft Office preferred.

Knowledge, Skills, and Abilities

· Knowledge of practical nursing principles, clinical procedures, medication administration, and infection prevention practices.

· Knowledge of HIPAA regulations and the ability to maintain the confidentiality, security, and integrity of patient information.

· Strong assessment, organizational, and critical thinking skills with the ability to prioritize patient care in a fast-paced clinical environment.

· Excellent customer service, communication, and interpersonal skills with a commitment to providing compassionate, culturally competent, patient-centered care.

· Proficiency in Microsoft Office and Electronic Health Record (EHR) systems.

· Ability to exercise sound clinical judgment, respond appropriately in emergency situations, and establish effective working relationships with patients, providers, leadership, and coworkers.
