Job Summary:

We are seeking a compassionate and skilled LPN to join our interdisciplinary team of healthcare professionals. As an LPN at Warren Nursing & Rehab, you will provide high-quality patient care, develop and implement individualized care plans, and work collaboratively with our medical team to ensure the best possible outcomes for our patients.

Responsibilities:

• Provide direct patient care, including administering medications, performing patient assessments, and developing care plans
• Work closely with our interdisciplinary team to ensure comprehensive care for our patients
• Develop and implement individualized care plans that address the unique needs and goals of each patient
• Maintain accurate and detailed records of patient care and treatment
• Participate in continuous quality improvement initiatives to improve patient outcomes and satisfaction
• Collaborate with our medical team to identify and address patient concerns and needs

Requirements:

• Current licensure as a Licensed Practical Nurse in the State of Ohio
• High school diploma or equivalent required
• 1-2 years of experience as an LPN in a skilled nursing facility or similar setting
• Basic Life Support (BLS) certification
• Ability to work effectively in a fast-paced environment and prioritize tasks to meet the needs of our patients
• Excellent communication and interpersonal skills
