Licensed Practical Nurse (LPN), General Surgery

US:PA:St Marys | Nursing & Nursing Support | Full Time Regular

As a Licensed Practical Nurse, you will assist in call center functions while performing duties in the areas of transitional care management and chronic disease management. You will support the physician or mid-level provider with assisting in development, implementation and evaluation of patient care in the office. He/she will be required to coordinate the care with other services or outside health care providers. The LPN will serve as a liaison between the physician, the mid-level provider and all other health care providers.

Qualifications:

  • High school diploma or equivalent required
  • Current LPN licensure in the state of Pennsylvania required
  • Knowledge in regulatory requirements of third party payors such as Medicare, Medicaid and Blue Shield preferred
  • CPR certification from the American Heart Association within the first year of hire required
  • Two years of office experience preferred.
  • Familiarity with Microsoft applications preferred
