Job description

Position: LPN (Contract to Perm)
Location: NY 14527
Pay: $1,326 – $1,495/week

Contract Duration:

  • 05/12/2026 – 08/15/2026 (13 weeks)

Schedule:

  • Part-Time Evenings (24 hours/week)

  • Rotating: 2x8-hour shifts / 3x8-hour shifts

  • Every Other Weekend (EOW) & Holidays as needed

  • Must be available for full-shift orientation on a Tuesday or Thursday


Position Summary & Responsibilities
  • Provide direct resident care within LPN scope for adult/geriatric patients

  • Perform ADLs: hygiene, dressing, toileting, positioning, skin care

  • Maintain safety: respond to call lights, fall prevention, monitor equipment

  • Assist with nutrition/feeding per care plan and resident preferences

  • Support mobility: transfers, ambulation, ROM, assistive devices

  • Monitor, assess, and report changes in resident condition

  • Complete accurate documentation per protocol


Requirements
  • Active NY LPN License (required)

  • BLS Certification (AHA or RQI only)

  • Minimum 1 year LPN experience (LTC preferred)

  • High School Diploma or GED

  • Ability to work with potential exposure to blood/body fluids
