DESCRIPTION

  • Provides professional social work services through the assessment and resolution of problems and issues that impact overall health and wellbeing, including discharge planning.

 

QUALIFICATIONS

  • Completion of a 4 year bachelor’s degree in Social Work or Masters of Counseling from an accredited college or university
  • Licensed Social Worker (LSW) or Counselor (LPC) in the State of Ohio
  • Extensive knowledge in human behavior and personality development
  • Ability to use a holistic approach to patient care
