Responsibilities
Essential Functions:
  • Deliver exceptional care to the dedicated client in their home
  • Help execute comprehensive personalized weekly schedules and care plans
  • Provide skilled nursing services as prescripted under a client's plan of care that can be safely provided in the home, within your scope of practice
  • Counsel clients and family in meeting nursing and related needs
  • Meet all documentation requirements according to agency policy and proecedure
  • Coordinate services by participating in case conferences, case management, team meetings, and staff meetings as indicated
  • Administrative functions such as documentation of completion of tasks and report changes in the client's physical condition, mental capability or behavior

Qualifications
