Job Summary:

This is a full-time position that covers two sites within Ascension WI in the Milwaukee area. The primary site is Ascension Columbia Saint Mary.

The Inpatient Hospital Diabetes Nurse Practitioner functions interdependently within the hospital environment providing direct and indirect holistic care. The Inpatient Hospital Diabetes Nurse Practitioner role focuses on glycemic management to include but not limited to assessment and evaluation of new consults, following up with existing consults, writing orders related to diabetes management including diabetes medications, labs and writing referrals as needed. The NP assessment includes the patient’s physical, psychosocial, education, rehabilitative and discharge needs. Within this role, they will work as part of a cohesive diabetes self-management education team and collaborate with the medical, nursing and ancillary hospital staff.

Role and Responsibilities:

Analyze and interpret patients’ histories, symptoms, physical findings, and diagnostic information to develop appropriate diagnoses

Develop treatment plans based on scientific rationale, standards of care and professional practice guidelines

Order outpatient follow-up to include diabetes education and/or specialist intervention when appropriate

Perform proper and timely billing for services rendered

Ensure that billing and clinical documentation adheres to compliance standards of host hospital

Develop and maintain collaborative physician relationship, ensuring effective communication, openness to feedback and recommendations for improvement as needed

Prescribe medications based on efficacy, safety, insurance coverage and cost as legally authorized

Counsel patients about drug regimens and possible side effects or interactions

Recommend interventions to modify behavior associated with health risks

Detect and respond to adverse drug reactions, with special attention to vulnerable populations

Educate patients about self-management of acute and chronic illnesses, tailoring instructions to patients’ individual circumstances

Communicate with interdisciplinary team to coordinate interventions, revise patient plan of care to meet changing needs and assure continuity of care

Provide coaching, mentoring and instruction to hospital bedside care providers

Resolves a wide range of issues in creative ways

Demonstrate a commitment to professional accountability and growth

Minimum Qualifications

Masters or Doctoral Nursing degree- Family or Acute Care Nurse Practitioner required

At least 2 yrs experience as a Nurse Practitioner

Current state license

DEA license

Board Certified in Advanced Diabetes Management BC-ADM credential (If not already credentialed, will be required to sit for the exam within 2 years of hire.)
