Physical Therapist (DDD Provider)

About FamAli LLC

FamAli LLC partners with Arizona’s Division of Developmental Disabilities (DDD) to deliver high-quality therapy services. We support individuals with developmental disabilities in achieving greater mobility, independence, and participation in daily life.
Key Responsibilities

  • Conduct DDD-compliant PT evaluations (gross motor, mobility, strength, balance, equipment needs).
  • Develop and implement Individualized Treatment Plans (ITPs) based on ISP goals.
  • Provide direct PT services in home, community, or facility settings.
  • Recommend and train on mobility aids, orthotics, and adaptive equipment.
  • Participate in ISP meetings and collaborate with DDD Support Coordinators.
  • Educate caregivers and support staff on therapeutic exercises and safety.
  • Maintain documentation per AHCCCS and DDD standards.
  • Monitor progress and adjust treatment plans as needed.
  • Stay current with Arizona PT licensure and DDD policies.
Qualifications

  • Doctorate or Master’s in Physical Therapy
  • Arizona PT license (required)
  • Experience with developmental disabilities (preferred)
  • Fingerprint Clearance Card, driver’s license, reliable transportation
  • Ability to meet DDD credentialing requirements
