Clinical Role:

Seeking an experienced Family Nurse Practitioner (FNP) or Physician Assistant (PA)
100% outpatient primary care practice
Monday - Friday, 8:00 AM - 5:00 PM
Minimal call responsibilities
Patient population is exclusively adults ages 65+
Average daily patient volume: 12-15
Value-based care model with a strong emphasis on quality outcomes and patient-centered care
Collaborative care team includes a full-time physician, nurse practitioners, medical assistants, and care coaches
Candidates who are fluent in Spanish are strongly preferred, as the practice serves a large Spanish-speaking patient population.
Minimum of two years of full-time experience practicing as a Nurse Practitioner in a primary care setting required (no new graduates)
Experience caring for geriatric patients (65+) strongly preferred
Experience in a value-based care environment is a plus
Athena EMR
DAX documentation software available
Focus is on delivering exceptional patient care—not RVUs or high patient volume
