Job description

We are working on an exciting new opportunity with a growing healthcare company that is seeking Nurse Practitioners or Physician Assistants in Tacoma, WA to work visiting patients at home and virtually. -Full time schedule in patient homes and at home: Monday through Friday 8:30 - 5:00 pm
-Home care or hospice experience preferred
-Chronic disease management experience preferred
-Must be willing to travel up to one hour from home - can live in Tacoma or Puyallup
-Must be open to travel (this could be quarterly, up to 3 days at a time and up to approx. 200 miles away)
-Must be willing to visit patients in their homes 4 days/week, 1 day remote during non-travel weeks
