DEPARTMENT SUMMARY: St. Charles Health System Community Care supports the health system mission by providing innovative care to achieve better health, better care, and better value. Community Care helps to bridge any gaps in the needs of patients as they navigate through the continuum of care, and throughout the life cycle, providing care and supportive services to the communities we serve. The services encourage a focus on providing care for post-acute conditions and outpatient care within the realm of Home Health, Hospice, and Palliative Care, while developing strong relationships, service line collaboration and referrals of higher acuity conditions. Care is provided to patients and families as part of an integrated system of settings, services, and care levels with the goal of helping our patients live as independently as possible with a higher quality of life, even and especially during advancing illness and end-of-life care.
POSITION OVERVIEW: The Support Services Coordinator is responsible for supervising and supporting daily needs of the Home Health and Lifeline departments, as well as Community Care needs when appropriate. This position does not directly manage caregivers.
ESSENTIAL FUNCTIONS AND DUTIES:
Monitors, processes and oversees various Epic work queues (e.g. face-to-face, discharges, insurance authorizations) to ensure timely completion of home health tasks.
Provides administrative support to Home Health and Lifeline department teams, as well as back-up to Hospice and AIM/Palliative Care teams.
Assists caregivers and leadership with Kronos/timecard issues and performs any other needed support to the team.
Uses purchasing system to order Home Health office supplies, forms, and equipment on an ongoing basis to meet department needs. Demonstrates fiscal responsibility for equipment and supplies of the department.
Coordinates Home Health medical supply process and is a resource to clinical staff for medical supply questions and online ordering system.
Oversees the coordination of Home Health Staff and Leadership, as well as other, meetings throughout the year.
Provides clerical support for all aspects of the hiring process, including scheduling orientation and education.
Assists Home Health Director and Managers by participating in special projects as required.
Ensures consistent and technical excellence and compliance, superior service, and the integration of the healing health care principles. Participates in annual plan including goals for areas supervised with director and managers.
Participates in budget development, accountability and meeting all operational targets for all areas within span of control.
Complies with all applicable laws, regulations, policies and procedures, supporting the health system's corporate integrity efforts by acting in an ethical and appropriate manner, reporting known or suspected violations of applicable rules, and cooperating fully with all company investigations and proceedings.
Maintains ongoing communication with staff, department leadership, and others to ensure needs are being met for staff and patients.
Supports the vision, mission and values of the organization in all respects.
Supports Value Improvement Practice (VIP- Lean) principles of continuous improvement with energy and enthusiasm, functioning as a champion of change.
Provides and maintains a safe environment for caregivers, patients and guests.

EDUCATION
Required: High school diploma or GED.
Preferred: Bachelor's degree in related field.
LICENSURE/CERTIFICATION/REGISTRATION
Required: Valid Oregon driver's license and ability to meet SCHS driving requirements. Ability to travel to business functions/trainings/meetings and all SCHS worksites.

EXPERIENCE
Required: Two years office management experience. Knowledge of Medicare/Medicaid regulations. Ability to handle multiple tasks and changing priorities.
Preferred: One year home health office management experience. Two years supervisory experience.

Must be able to wear appropriate Personal Protective Equipment (PPE) required to perform the job safely.
