At Legacy Home Health (a Ventana Ventures company), we are honored to care for individuals who served our country by previously working for the Department of Energy in various capacities and who developed illnesses related to radiation, beryllium, or silica exposure. These patients qualify for care through the Department of Labors Energy Employees Occupational Illness Compensation Program Act (EEOICPA). Our mission is to provide compassionate, dependable, and personalized care that allows patients to remain safe and comfortable in their homes while receiving high-quality medical support.

We are currently seeking a compassionate and dependable Registered Nurse (RN) to join our team and provide skilled nursing care to patients in their homes. This role allows nurses to build meaningful one-on-one relationships with patients while delivering individualized care in a comfortable home setting. Responsibilities & Day-to-Day Duties

**We are currently looking for a nurse who is available one day a week for 4-6 hours.

Responsibilities of the Registered Nurse (RN) may vary based on patient needs and individualized care plans but can include:

  • Conducting patient assessments and monitoring overall health status
  • Developing, implementing, and updating individualized care plans
  • Administering medications and monitoring for side effects or adverse reactions
  • Coordinating care with physicians, caregivers, family members, and interdisciplinary healthcare teams
  • Monitoring vital signs and documenting patient progress
  • Providing skilled nursing care and treatment as ordered by physicians
  • Educating patients and families regarding medications, treatments, safety, and disease management
  • Assisting patients with activities of daily living when needed
  • Evaluating patient safety within the home environment
  • Monitoring for changes in condition and reporting concerns appropriately
  • Maintaining accurate and timely nursing documentation and visit notes
  • Supervising and collaborating with caregivers or home health aides involved in patient care
  • Ensuring compliance with company policies, state regulations, and patient confidentiality standards
  • Providing emotional support and compassionate companionship to patients and families
  • Assisting with care coordination and communication between all members of the care team
  • Responding appropriately to urgent patient concerns or changes in condition


Qualifications

  • Current and active Registered Nurse (RN) license in the state of employment
  • Previous home health, hospice, long-term care, or clinical nursing experience preferred
  • Strong assessment and critical thinking skills
  • Ability to communicate effectively with patients, families, physicians, and healthcare staff
  • Must be able to read, write, and communicate in English
  • Ability to complete accurate documentation and reports in a timely manner
  • Must be able to follow verbal and written instructions carefully
  • Reliable transportation and valid drivers license required
  • Ability to work independently and manage time effectively
  • Must maintain professionalism, confidentiality, and respect for patient privacy
  • Current CPR certification required
  • Must pass a background check
  • Physical Requirements
  • Ability to stand, walk, bend, kneel, and assist patients throughout shifts
  • Ability to lift, transfer, or reposition patients when necessary
  • Ability to travel between patient homes as needed
  • Ability to respond appropriately in emergency situations
