Job description

MINIMUM QUALIFICATIONS

Education – Incumbent must be a graduate of an accredited professional school of nursing. BSN preferred.

Experience – Prefer one year of critical care, emergency, or supervisory experience.

Licenses & Certification –Incumbent is required to possess current licensure by the State of Oklahoma or National Council of State Boards of Nursing for Registered Nurse. BLS, ACLS, and PALS certifications required, or within 60 days of hire.

Knowledge & Skills –

  1. Professional knowledge of nursing care principles, quality management, ethics, infection prevention practices and processes required to assess nursing needs of patients and provide education to nursing staff.
  2. Knowledge of a wide variety of medical disorders, normal course of diseases, potential complications and individual therapeutic interventions.
  3. Knowledge of pharmaceuticals, their desired effects, side effects and complications of their use.
  4. Ability to recognize and react to emergency situations.
  5. Knowledge and skill in operation and monitoring of general nursing equipment, e.g. infusion pumps, defibrillators, arrhythmia monitors, etc.
  6. Ability to triage and assign resources based on patient clinical needs and acuity.
  7. Ability to supervise and evaluate nursing staff.
  8. Knowledge necessary to manage a fiscal budget.
  9. Computer literacy and ability to become proficient on electronic medical record.
  10. Ability to communicate professionally and effectively in oral and written forms with providers, nurses, patients, and hospital administration.
JOB SUMMARY

The position serves as an integral part of the Hospital operations team by coordinating patient transfers throughout the health system. This position requires daily high-level communication to providers and clinical support staff and ensures that patients are placed in the appropriate level of care available at each unique facility. This position also ensures that patients are cared for inside of the health system when clinically appropriate. Incumbent is under the supervision of the RN Transfer Coordinator Supervisor.

WORK ENVIRONMENT

Work is performed in a hospital environment. There is common exposure to contagious disease. The work may include a certain amount of exposure to hostile and emotionally disturbed patients, families and visitors. Special attire may be required as protection against infection.

PHYSICAL DEMANDS

Long periods of standing and walking. Working with patients requires recurring bending, lifting, squatting, stretching and positioning of patients. Must be able to lift up to 50 pounds. Must manage stress under many different circumstances.

ESSENTIAL FUNCTIONS

Satisfactory job performance will be determined by successful execution of the following:

  1. Maintains RN core competencies.
  2. Consults other departments, as appropriate, to provide for an interdisciplinary approach to the patient’s needs. Treats patients and their families with respect and dignity. Identifies and addresses cultural and psycho-social needs of patients and their families.
  3. Coordinates with nursing leadership, house supervisors and providers throughout the health system to facility patient transfers and patient movement to provide the right care, at the right time, in the right location.
  4. Conducts the assessment and reassessment of all patients to ensure appropriate specialty services are being rendered and ensure the correct level of care is met in each healthcare location.
  5. Maintains knowledge of MCNDH policy and procedures and EMR functionality related to patient movement and transfers.
  6. Maintains all provider schedules needed to facilitate patient movement, including call schedules and specialty services.
  7. Performs daily review of each hospital’s staffing and resources, communicates issue using chain of command.
  8. Addresses and reports patient and/or staff complaints using chain of command with a goal to resolve all complaints at the time of service.
  9. Provides support for “Code Blue” and “Rapid Response” team by evaluating critical care resources and facilitating patient movement to appropriate level of care.
  10. Maintains a professional demeanor and promotes professional, ethical and legal standards of nursing.
  11. Regular attendance and professional attire required.
  12. Performs other duties as assigned.
