Fairport Baptist Homes Adult Care Facility is seeking a compassionate and detail-oriented UAS Certified Registered Nurse (RN) to join our assisted living team on a part-time basis. The UAS Registered Nurse (RN) role focuses on conducting Uniform Assessment System (UAS) evaluations for residents, supporting individualized care planning, Medicaid eligibility processes, and regulatory compliance. The ideal candidate has experience in Assisted Living Programs (ALP) and is committed to delivering high-quality, resident-centered care.

UAS Certified Registered Nurse (RN) Key Responsibilities:

  • Conduct timely and accurate UAS assessments for assisted living and ALP residents

  • Evaluate residents’ physical, cognitive, and psychosocial needs while supporting Medicaid documentation requirements

  • Collaborate with interdisciplinary teams to develop and maintain individualized service plans

UAS Certified Registered Nurse (RN) Qualifications:
