• Conduct thorough clinical assessments to evaluate mental and emotional health, leading to tailored treatment plans that address individual patient needs.
• Implement evidence-based therapeutic interventions, utilizing methodologies such as solution-focused therapy and cognitive-behavioral techniques to assist patients in managing grief and emotional distress.
• Maintain accurate and timely clinical documentation, ensuring compliance with healthcare standards and regulations while minimizing unnecessary paperwork.
• Collaborate with a multidisciplinary team including physicians, nurses, and healthcare aides to develop integrated and holistic care plans.
• Provide education and emotional support to patients and their families, guiding them through difficult decisions and enhancing their coping mechanisms.
• Stay current with the latest research and developments in mental health, psycho-social interventions, and hospice care practices to continuously improve efficiency and care quality.

In this role, you will work primarily with adults and their families in hospice care, many of whom may be experiencing significant grief, anxiety, and depression associated with terminal illness. This patient population requires not only clinical expertise but also sensitivity and an understanding of the unique emotional and spiritual challenges they face. Your work will play a crucial role in improving their quality of life during this critical time, making your contributions deeply meaningful.

• A Master's degree in Social Work from an accredited institution.
• Possession of a valid Licensed Clinical Social Worker (LCSW) license in the state of Illinois.
• Proficiency in psychotherapeutic methods, particularly solution-focused therapies and crisis intervention techniques.
• Strong communication and interpersonal skills, with the ability to connect meaningfully with diverse populations.
• Active listening skills and a compassionate demeanor tailored towards healthcare settings.

• Minimum of two years of direct experience in a mental health or clinical setting, preferably with hospice or palliative care exposure.
