Job description

We are seeking a skilled and compassionate Registered Nurse to provide specialized in-home training for patients and their caregivers on performingperitoneal dialysis (PD) safely and independently. This role focuses on patient education, hands‑on instruction, ongoing support, and home visits to ensure adherence to treatment protocols, prevent complications, and promote quality of life for individuals with end‑stage renal disease (ESRD).

Key Responsibilities
  • Deliver comprehensive, hands‑on PD training, including techniques such as continuous ambulatory peritoneal dialysis (CAPD) and automated peritoneal dialysis (APD).
  • Teach aseptic procedures, catheter care, solution exchanges, machine operation, troubleshooting, and infection prevention.
  • Adapt training methods to meet the learning needs of patients, family members, and caregivers.
Home Assessments & Follow‑Up
  • Conduct initial and periodic home visits to assess environmental suitability, supplies, and patient technique.
  • Provide ongoing support through follow‑up visits, phone consultations, and troubleshooting for potential complications (e.g., peritonitis, catheter issues).
Case Management & Care Coordination
  • Monitor patient progress through lab results, vital signs, and treatment adherence.
  • Collaborate with multidisciplinary healthcare professionals—including nephrologists, dietitians, and social workers—to develop and maintain individualized care plans.
Quality, Safety & Compliance
  • Maintain regulatory compliance with all relevant nursing qualifications and home dialysis standards.
  • Accurately document patient assessments, training sessions, and care activities in electronic medical records (EMR).
  • Report and address any incidents or deviations from treatment protocols.
Qualifications
  • Active Registered Nurse (RN) license.
  • Experience in peritoneal dialysis (PD) training.
  • Minimum of one year of nursing experience, including hemodialysis and peritoneal dialysis exposure.
  • At least six months of general dialysis experience.
  • Certified Dialysis Nurse (CDN) preferred.
  • Current BLS/ACLS certification.
