Licensed Practical Nurse (LPN)

We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our nursing team. As an LPN, you will play a vital role in providing high-quality resident care under the direction of attending physicians and our Medical Director. Responsibilities:

· Provide direct nursing care to residents, including medication administration, wound care, and vital sign monitoring.

· Observe, assess, and report changes in resident condition to physicians and the nursing team.

· Assist in the development and modification of resident care plans.

· Document resident care accurately and thoroughly in electronic health records.

· Collaborate with physicians, nursing staff, and other departments to ensure comprehensive resident care.

· Participate in facility surveys and quality improvement initiatives.

· Maintain a safe and therapeutic environment for residents.

Qualifications:

· Current, unencumbered LPN license.

· Ability to make independent decisions and exercise sound judgment.

· Strong communication and interpersonal skills, with the ability to interact tactfully with residents, families, and staff.

· Knowledge of nursing and medical practices, procedures, and long-term care regulations.

· Ability to work effectively in a team environment and supervise other personnel as needed.

· Patience, compassion, and a positive attitude.

· Ability to lift, push, pull, and transport residents with assistance as needed.

Working Conditions:

· Fast-paced environment with frequent interruptions.

· Exposure to infectious waste, diseases, and hazardous chemicals (PPE provided).

· May require working beyond normal hours, including weekends and holidays.
