Introduction:
Family Ties Home Care has an opening for a client in Woodland Hills who needs live in care. She has mild to moderate cognitive decline and walks with a walker and needs stand by assistance. Needs help with ADL's.

Responsibilities:
  • Following a pre-set care plan
  • Standard ADLs (personal care, bathing, dressing, grooming, hygiene assistance)
  • Assist with medication reminders
  • Monitor and report on client's well-being
  • Light housekeeping, laundry, & other household daily chores as needed
  • Grocery shopping & errands
  • Cooking/meal preparation
  • Scheduling appointments, including doctor's
  • Communication with Family Ties HC management & family members/loved-ones regarding the client's daily status
  • Daily notes
  • Other duties assigned as needed

Qualifications:
  • Current HCA or higher certification
  • Caregiving experience of 1 year or similar experience
  • Compassionate and caring demeanor
  • Excellent communication & interpersonal skills
  • Ability to follow the pre-set care plan and medication schedule
  • Ability to lift 35 pounds
  • Ability to assist with client's mobility as needed
  • Valid driver's license and reliable transportation
  • Ability to work the schedule as described
