Home Health Aide

The Home Health Aide role is primarily responsible for assisting in the implementation of the Lifespark Experience and the Life Care Management model including but not limited to assisting with Life Plan, Care Assignment activities, and delegated duties as well as honoring clients' rights, customer service and communication, emergency preparedness, and safety.

Schedule: Shifts are 4-hours minimum, live-in shifts available. 1 and 2-hour visits available Wage: $18.00 - $24.00 – plus mileage reimbursement 72.5 cents per mile Multiple openings: Minnetonka, Wayzata, Chanhassen, Eagan, Apple Valley, Burnsville, St. Paul, Woodbury, White Bear Lake, Lino Lakes

Position Functions and Responsibilities

Lifespark Experience

  • Assist with implementation of Lifespark Experience including items identified on the Life Plan
  • Review Life/Care Plan and understand expectations
  • Provide companionship and socialization
  • Assist with activity and hobby fulfillment
  • Provide transportation (as needed)
  • Other Duties as Assigned
  • Assist clients with activities of daily living and delegated tasks according to clients' care plan, needs, and wishes.
  • Review Care Plan and follows what is expected
  • Personal Cares (bathing, dressing, grooming, toileting, skin care, etc.)
  • Lifting, Transferring (pivot transfer, 2-person, Hoyer lift)
  • Range of Motion & Positioning
  • Exercise & Ambulation
  • Meal Prep and assist with feeding (as needed)
  • Homemaking (light housekeeping, laundry, personal shopping)
  • Vital Signs
  • Medication Reminders and Administration
  • Special Tasks as directed by LCM
  • Observes and Reports Client Status updates and changes
  • Follows all documentation requirements
  • Other Duties as Assigned

To gauge a Caregivers' effectiveness results are measured using the following indicators and targets:

Quality & Compliance

  1. Completion of documentation including clocking in, clocking out, and task completion for all visits
  2. Completion of all new hire and annual in-service requirements

  • Participation in team meetings unless approved by supervisor

  • Willingness to work beyond scheduled shift if necessary to fulfill client need.
