Thrive Homes is a recognized provider of Family Model services for DDA Waiver and ECF/Choices Enrollees. We are dedicated to creating a supportive and inclusive environment for individuals with disabilities. We are looking for compassionate and dedicated individuals to join us in this important role of providing a nurturing home and comprehensive care to our program participants.
Key Responsibilities:

As a Family Model Provider, you will open your heart and home by offering room and board to one or two individuals with disabilities. You will be responsible for providing person-centered direct personal care, including activities of daily living such as cooking, bathing, cleaning, medication administration assistance, and accompanying individuals to medical appointments. Your support will play a crucial role in helping the individuals thrive, achieve independence, and work towards their personal goals and outcomes outlined in their PCSP (Person-Centered Support Plan).
Qualifications and Experience:

  1. Minimum of 1 year of experience working with individuals with intellectual disabilities.
  2. Clear criminal background check is a must.
  3. Access to a safe and accessible home or apartment with a dedicated bedroom for the individuals, especially if wheelchair accessibility is required.
  4. Reliable vehicle and valid auto insurance and driver's license for transportation.
  5. Completion of all required training, including Relias, Medication Administration Certification, CPR and First Aid.
  6. Access to a personal computer with internet connectivity for documentation purposes.
  7. High school graduate or equivalent education preferred.
