Posting Title: Home Health Aide (HHA) / Caregiver
You'll join a compassionate, fast-growing home health and home care team that supports patients one-on-one in their homes and communities. Expect to be hands-on - providing personal care, supporting daily living needs, and making a real difference in someone's day, every shift. This is a role for someone dependable, patient, and service-driven who wants meaningful work and flexible opportunities. Licensed HHAs/CNAs are encouraged to apply - and we also welcome motivated candidates willing to become certified.

Key responsibilities
  • Provide personal care support including bathing, grooming, dressing, oral hygiene, and toileting
    โ€ข Assist with mobility, transfers, ambulation, and range-of-motion exercises per care plan
    โ€ข Prepare meals and assist with feeding as needed
    โ€ข Support medication self-administration through reminders (no independent med administration)
    โ€ข Monitor and report patient condition and changes to the supervising nurse
    โ€ข Take and record basic vital signs when trained and assigned
    โ€ข Maintain a clean, safe, and orderly client environment
    โ€ข Assist with linens and light client laundry as appropriate
    โ€ข Use safety equipment properly (rails, supports, stools, etc.)
    โ€ข Document care provided according to company and regulatory standards
    โ€ข Travel reliably to patient homes and assigned visits across your service area
    โ€ข Complete required in-service training, including annual dementia education
