Job description

Position Summary: Responsible for supporting the mission of the hospital. Facilitates patient assessments and admissions to the Lighthouse Intensive Outpatient Program as assigned. Provides therapeutic services to groups, individuals, and families participating in the Lighthouse Intensive Outpatient Program as assigned. Responsible for Bio-psychosocial as assigned. Actively adds and addresses problems on the Master Treatment Plan and participates in Treatment Team meetings. Collaborates with team members to write Discharge Plan for patients and meets with patients at discharge when needed. May at times need to provide therapy to individuals and/or families who are inpatient within the hospital. Works collaboratively withinterdisciplinary team which includes other staff, physicians, and other hospital departments. Participates in community outreach activities as needed. Utilizes knowledge of standards, policies and procedures as outlines for the hospital.
Position Qualifications:
  1. Licensed in the State of Texas as a Licensed Professional Counselor (LPC)
or a Licensed Marriage and Family Counselor (LMFT)
  1. Experience in providing therapeutic services in a geriatric behavioral health setting preferred
  2. Experience providing group, individual, and family therapies.
  3. Ability to provide therapeutic services to ensure patient safety and satisfaction and promote the integrity of program.
  4. Maintains and displays a positive attitude at all times.

Essential Functions:
  1. Maintains patient confidentiality as outlined in HIPPA and program policies and guidelines.
  2. Has specialized knowledge and understands the challenges of the geriatric population.
  3. Interviews patient and completes patient assessment as needed.
  4. Completes patient bio-psychosocial according to timeline established in policies and procedures as directed.
  5. Facilitates group therapy, as well as individual and family therapies.
  6. Maintains patient records and documents according to hospital policies and regulations.
  7. Writes reports and/or evaluations when needed.
  8. Guides patients in the development of skills and or strategies for dealing with their problems. Teaches life skills to patients or their families.
  9. Communicates effectively and in a timely manner with referral sources, physicians, and families when appropriate.
  10. Is comfortable intervening in a crisis situation to assist patients.
  11. Adds problems to Master Treatment plan and monitors patients and evaluates treatment progress.
  12. Works with interdisciplinary team to develop and implement successful discharge plan for each patient.
  13. Participates in Community Awareness Activities as assigned.
  14. Leads classes or community events. Plans programs to address community health issues as needed and/or directed.
  15. Collaborates with treatment team to develop and facilitate effective and successful discharge plans for each patient.
  16. Continually learns about new developments in counseling by reading professional literature and attending courses, workshops and seminars.
  17. Represents Golden Plains Community Hospital and the Lighthouse program in a positive manner at all times.
