Job description


Family Guidance Centers, Inc. (FGC), a not-for-profit behavioral healthcare organization, treats and prevents substance use disorder, as well as an array of other related problems. Family Guidance Centers, Inc seeks to support the behavioral health of individuals, families, and communities through our commitment to innovation, accountability, and compassionate care to facilitate positive, lasting change.
Essential Duties and Responsibilities:
  • Works as part of a multi-disciplinary team of the hospital staff and FGC staff responsible for assisting, monitoring and coordinating services for Medical Detox Patients.
  • Assists patients with a variety of concerns, but the primary focus of the Medical Detox Unit is for patients to be bio-medically stabilized and to be referred to MAT and or a HLOC if requested.
  • Assists in performing and completing a comprehensive bio-psychosocial assessments, the Lighthouse Locator and Admission and Discharge and The Government Performance and Results Act on each patient during shift.
  • Assists with patient continued stay review process with the established procedure to monitor the medical necessity and appropriateness of continued hospitalization.
  • Works collaboratively in formulating an appropriate and realistic discharge care plan, communicating with the patient. Completes appropriate forms, and contracts all appropriate individuals to facilitate the transfer/linkage of patients to other community based agencies and services. Ensures all referrals are linked through follow-up.

Schedule: Saturday & Sunday (7:30a - 4:00p)
Start Date: ASAP
Pay Rate: $19.00 - $21.00 per hour; based on experience

Experience and Skills
Education and/or Experience: High School Diploma / GED required; Experience in/with substance use, detox programs, medication-assisted treatment, behavioral healthcare and/or community relations encouraged to apply.
Certification, Licensure, Registrations: IAODAPCA Certified Alcohol and Other Drugs Counselor (CADC) preferred
