We are specifically seeking candidates with experience supporting individuals with intellectual/developmental disabilities in community employment settings, including DD Waiver supported employment/job coaching services.

LifeROOTS is a nonprofit organization that provides services that promote independence and community inclusion for individuals with developmental disabilities and other support needs. We are seeking compassionate, motivated, and reliable professionals to join our Supported Employment team as a Job Coach.
This position is ideal for someone with experience supporting individuals with intellectual and developmental disabilities in community employment settings, including DD Waiver supported employment services.
This role also includes participant support coordination, documentation, communication with interdisciplinary teams, and assistance with ISP-related service activities.
This is not a remote position. This is a community-based role that requires daily travel throughout Albuquerque to support participants at job sites and in the community.
What You'll Do
• Provide on-site job coaching and employment support to individuals with developmental disabilities in community workplaces
• Teach and reinforce job tasks using task analysis, prompting, modeling, and fading supports
• Help participants build independence, workplace confidence, communication skills, and successful work habits
• Support participants in maintaining competitive integrated employment aligned with their vocational goals
• Assist with participant support coordination, service documentation, and ISP-related activities
• Communicate professionally with employers, coworkers, families, guardians, service coordinators, and interdisciplinary team members
• Assist with workplace accommodations and identify strategies that support participant success
• Help develop natural workplace supports and promote workplace inclusion
• Participate in ISP and IEE implementation and documentation
• Complete required documentation accurately and timely in accordance with DD Waiver standards
• Collaborate with supervisors and team members regarding participant progress and support needs
• Support participants in both short-term and long-term employment settings
• Maintain professionalism, confidentiality, and ethical conduct at all times

Requirements
Job Duties / Principal Accountabilities
• Provide on-site job coaching and employment support to participants in community-based employment settings.
• Support participants in obtaining, learning, and maintaining community intergrate employment.
• Teach job tasks using task analysis, modeling, prompting, redirection, repetition, fading supports, and other job coaching strategies.
• Observe and monitor participants in employment settings to assess work performance, independence, safety, communication, and support needs.
• Assist participants with developing positive work habits, workplace communication, professional behavior, attendance, time management, and task completion.
• Work side by side with participants at job sites when needed while promoting independence and reducing support over time.
• Build natural supports between participants, employers, supervisors, and coworkers to promote long-term workplace success.
• Communicate professionally.
• Participate in the development, implementation, and monitoring of participant employment goals.
• Complete required documentation accurately and timely, including service notes, ISP-related documentation, DQD documentation, progress updates, and other program records.
• Identify workplace barriers and assist with problem-solving before concerns become a crisis for the participant, employer, or coworkers.
• Support workplace accommodations, self-advocacy, assistive technology, and ADA-related needs as appropriate.
• Upon certification, participate in job development and employment retention activities in coordination with the Employment Services Manager.
• Maintain compliance with DD Waiver, Mi Via, State General Fund standards, LifeROOTS policies, and applicable program requirements.
• Maintain professional and ethical conduct, confidentiality, and appropriate boundaries at all times.
• Present LifeROOTS and the Supported Employment program in a positive and professional manner to employers, community partners, participants, and the public.
• Attend required meetings, trainings, and continuing education activities.
• Complete required paperwork and electronic documentation according to departmental procedures and deadlines.
• Provide limited personal assistance support when necessary, based on participant needs, safety, and program requirements.
• Participate in department quality improvement, health and safety, and compliance activities as assigned.
• Cross-train with Community Integration Coach responsibilities as needed to support participant services and department coverage.
• Perform other related duties as assigned by the Employment Services Manager.
Salary Description
$19 per hour
