Job Summary:

We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to provide basic medical attention to patients and assess their needs. As an LPN at Mountainside Nursing Home, you will play a vital role in the day-to-day coordination and oversight of the nursing process for our residents.

Responsibilities:
  • Provide direct and indirect provision of care according to each resident's nursing and medical plan of care and in accordance with state and federal regulations and accepted nursing standards of practice
  • Assess, observe, and evaluate resident's clinical condition, documenting findings in resident electronic medical record and notify the provider and/or RN when necessary
  • Update or modify care plans as needed to reflect any changes to a resident's care and service needs
  • Administer medications timely, accurately, and according to orders
  • Participate in meetings related to resident outcomes and care
  • Respond to residents, family, or responsible party in a timely and professional manner
Requirements:
  • 1 year of experience as a Licensed Practical Nurse (LPN)
  • Current LPN license in the state of New Jersey
  • Ability to work in a fast-paced environment and prioritize tasks effectively
  • Strong communication and interpersonal skills
