Job Description
Commits to a culture of safety and promotes an environment that encourages event identification, reporting, remediation, and prevention through education, system redesign, and/or process improvement. Works with executive leaders proactively in developing system strategies to mitigate the risk of harm and reactively to improve processes and prevent recurrence. Coordinates efforts of clinical operational leaders and frontline staff and works collaboratively for the purpose of measurably improving medication safety and reducing events.
Experience
  • 5 years of experience in hospital pharmacy or clinical

Preferred Qualifications
  • Experience in pediatrics
  • Experience in medication safety and quality methodology
  • Knowledge of Joint Commission and other applicable regulatory standards

Education
  • Bachelor of Science in Pharmacy or PharmD required

Certification Summary
  • Licensed registered pharmacist in the state of Georgia

  • Ensures medication safety risk reduction strategies are present across the continuum of care.
  • Participates as a member of the quality leadership team to develop system quality goals and other quality initiatives.
  • Participates actively on the system PNT.
  • Actively participates in multidisciplinary Pharmacy Informatics Council to evaluate, design and promote safer Epic medication clinician decision support.
  • Provides management of system medication events utilizing detailed steps outlined in the patient safety event response process.
  • Designs and implements initiatives to promote and sustain a just culture of safety and high reliability, including communication, training, tools, and recognition.
  • Collaborates with accreditation and system leaders to ensure organizational compliance with regulatory standards related to medication safety.
  • Provides system collaboration regarding development of medication safety education, such as annual competencies, safety alerts, and simulation training.
  • Oversees systemwide infusion pump processes and drug library.
  • Works collaboratively with the pharmacy director to support the system Adverse Drug Event-Hospital Acquired Condition (ADE HAC) workgroup by identifying system opportunities that drive best practice and effective mitigation strategies.
  • Oversees bar code medication administration compliance.
  • Collaborates with affiliation team to evaluate affiliation hospital readiness, develop actions and monitor core standards.
  • Conducts proactive organizational medication safety related assessments.
