What is a liaison?

Liaisons are professionals who act as intermediaries or communication links between different groups, organizations, or departments. Their primary responsibility is to facilitate collaboration, resolve misunderstandings, and ensure the smooth exchange of information. Liaisons work in various industries, including healthcare, education, business, and government, adapting their approach to the specific needs of the entities they connect. They often coordinate projects, relay feedback, and help build strong working relationships to achieve common goals.

How does a liaison typically facilitate communication between different departments or organizations?

A Liaison acts as a bridge by regularly meeting with stakeholders from various departments or external partners to ensure everyone is aligned on objectives and expectations. They clarify misunderstandings, relay important updates, and advocate for their team's needs, which helps prevent miscommunication and project delays. This role often involves attending cross-functional meetings, preparing status reports, and proactively identifying potential areas of collaboration or conflict. Effective Liaisons are skilled at building relationships and adapting their communication style to suit diverse audiences.

What are the key skills and qualifications needed to thrive as a liaison, and why are they important?

To thrive as a Liaison, you need strong communication, relationship-building, and problem-solving skills, often supported by a relevant degree such as business, communications, or public relations. These skills are crucial for effectively connecting diverse groups, ensuring smooth collaboration, and achieving organizational goals.

How do you become a liaison?

To become a liaison, you typically need relevant experience in communication, negotiation, or a specific industry, along with strong interpersonal skills. Many roles require a bachelor's degree and proficiency with tools like email and collaboration software; certifications in conflict resolution or project management can also be beneficial.

Is a liaison a stressful job?

A liaison role can be stressful due to the need to manage communication between different groups, meet deadlines, and resolve conflicts. The level of stress varies depending on the industry, workload, and organizational environment, but strong interpersonal and organizational skills help manage job pressures.

Job description

Clinical Liaison

Starting at $85K/year

LPN or RN Preferred

Covering Portland and Hillsboro, Oregon area!

Turn your job into a career with Evergreen Healthcare Group!

Are you a nurse or clinically experienced healthcare professional ready to make a meaningful impact beyond bedside care? We’re looking for a compassionate, relationship-driven Clinical Liaison to join the Evergreen Healthcare Group team — serving skilled nursing facilities in the Portland/Hillsboro market.

In this regional role, you’ll serve as the key connection between hospitals, families, and our facilities — helping patients transition to post-acute care with confidence and clarity.

Why This Role Stands Out:

  • Starting salary: $85,000K

  • Mileage reimbursement for all work travel

  • Private healthcare (Medical, Dental, Vision)

  • “Work Today, Get Paid Tomorrow” with UKG Wallet

  • Professional development & training opportunities

  • Supportive leadership and room to grow

You’ll play a critical role in coordinating seamless admissions, building trust with healthcare partners, and ensuring patients receive the care they need in a setting that supports healing and quality of life.

Key Responsibilities:

  • Develop and maintain relationships with hospitals, physicians, discharge planners, and case managers

  • Evaluate patient referrals and assist with clinical screenings and pre-admission assessments

  • Coordinate admissions and ensure timely, efficient transitions from acute to post-acute care

  • Serve as a clinical resource and liaison to families and referral sources

  • Collaborate with internal facility teams to align care plans and meet resident needs

  • Track referral activity and participate in census development initiatives

Qualifications:

  • Active LPN or RN license (preferred) or clinical background in healthcare, case management, or marketing

  • Experience in skilled nursing, post-acute, or hospital-based discharge planning preferred

  • Excellent communication, organization, and relationship-building skills

  • Ability to work independently and travel regularly between facilities

  • Must reside within commuting distance of Portland/Hillsboro, Oregon

At Evergreen, we believe in creating a culture that values compassion, accountability, and growth — for both our residents and our team members. If you're ready to bring your clinical expertise to a role where relationships and outcomes matter most, we’d love to hear from you!

Evergreen Healthcare Group is an Equal Opportunity Employer.
