• Develop and implement comprehensive support packages customized to address clients’ unique challenges and dependencies.
• Conduct interviews with service users to assess their current emotional and social conditions, as well as understanding their needs, strengths, and weaknesses.
• Utilize a holistic approach to evaluate each case individually, allowing for the setting of tailored, measurable goals that promote client growth and recovery.
• Continuously monitor and assess clients’ progress, adapting treatment plans as necessary to meet evolving needs.
• Provide insightful information and counseling during therapeutic sessions, guiding clients in exploring actionable pathways to improvement.
• Maintain meticulous records of client interactions and assessments, ensuring accurate documentation of their progress and any changes in treatment status.
• Participate in case conferences and collaborately work with other professionals, offering evidence and recommendations when necessary.
• Act as a primary contact for clients, fostering strong relationships and engaging effectively with a multidisciplinary team.
• Refer clients to additional community resources and services to support their recovery and integration back into society.
