JOB DESCRIPTION

POSITION: PROGRAM MANAGER FLSA STATUS: EXEMPT

DEPARTMENT: PROGRAMS & SERVICES DIRECT REPORTS: NONE

REPORTS TO: DIRECTOR OF PROGRAMS & SERVICES

SUMMARY:

Provides case management/housing services to homeless individuals with a serious mental illness, chronic substance abuse, HIV/AIDS, fleeing domestic violence, medically fragile or dual diagnosis. This effort is done in collaboration with Permanent Supportive Housing team and social service agencies with whom Anawim Housing contracts.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

The following duties are normal for this position. These are not to be construed as exclusive or all inclusive. Other duties may be required and assigned.

  • Utilizes a framework of Motivational Interviewing, Stage Matched, De-escalation, Person Centered, and other practices to partner with program participants to achieve housing stability.
  • Works with participant and mainstream agencies in development of a person centered case plan and goals for each participant.
  • Collaborates with Centralized Intake in referral of program participants.
  • Assists each participant in locating an appropriate unit, which will meet their housing needs.
  • Facilitates the intake process, sublease, participant obligations and the level of monitoring for each participant in working to meet their case plan goals.
  • Locates scattered site rental properties and arranges for lease/sublease to maximize the grant funds.
  • Facilitates problem solving with participant and property manager on issues related to participant tenancy.
  • Completes home visits of program participant units to ensure ongoing monitoring and residential stability standards are met.
  • Maintains Homeless Management Information System data entry for program participants.
  • Maintains necessary file records in compliance with HUD regulations.
  • Follows the Iowa/Landlord tenant laws to resolve tenancy issues.
  • Maintains on-going communication with participant, landlords and any involved mainstream service agencies to ensure long term housing stability.
  • Works with mainstream service agencies in developing a resource list of employment opportunities for participants.
  • Conducts move-in and move-outs with participant and property manager.
  • Assures all work performed is done in accordance with OSHA safety standards and those established by Anawim Housing.
  • Assures that all work performed meets Anawim Housing’s standards for customer service, accuracy, quality and efficiency.
  • Establish and maintain an effective working relationship with employees, vendors, clients, tenants, equity partners and the financing authority.
  • Other duties as assigned.



MINIMUM QUALIFICATIONS:

  • Preferred BA/BS degree in Human Services or other related fields.
  • Experience with management of an apartment complex or homeless shelter is preferred.
  • Experience with population of homeless, mentally ill, substance abuse, HIV/AIDS, medically fragile or dual diagnosis as well as health care providers is preferred.
  • Familiarity with case management systems.
  • Requires knowledge of treatment issues and the ability to coordinate housing and mainstream service systems to work effectively to deal with those issues.
  • Uses computer for word processing and data entry.
  • Needs good verbal and written communication skills in dealing with participants, mainstream service agencies, landlords and other entities in the network of housing and services to homeless.
  • Requires basic mathematical and programming knowledge.
  • Valid driver’s license and good driving record.


ENVIRONMENTAL CONDITIONS:

The work is performed in a typical office or administrative environment as well as tenant apartments and outside. The worker could be exposed to adverse environmental conditions.


PHYSICAL REQUIREMENTS:

  • Driving, climbing, stooping, kneeling, crouching, walking, lifting, reaching, talking, hearing, grasping and finger dexterity, repetitive motions.
  • Sitting approximately 50 percent of the time.
  • Standing approximately 20 percent of the time.
  • Walking approximately 20 percent of the time.
  • Driving approximately 10 percent of the time.
  • Requires the exertion of up to 10 pounds of force continuously.
  • Up to 50 pounds of force infrequently to lift or otherwise move objects.

VISION REQUIREMENTS:

The minimum standard for use with those whose work deals largely with preparing and analyzing data and figures, accounting, transcription, computer terminal, and extensive reading.
