As an LCSW, you will collaborate with an interdisciplinary healthcare team that includes psychologists, nurses, and medical doctors. • Engage in the development of individualized treatment plans, including discharge plans, based on comprehensive psychosocial assessments and accepted social work interventions.
• Conduct thorough assessments to identify patients' needs and perform targeted interventions, maintaining accurate, timely records as required by department policies.
• Provide counseling and crisis intervention for patients who are experiencing emotional and behavioral challenges, ensuring their immediate safety and well-being.
• Consult with families and support systems to facilitate effective treatment and discharge planning, fostering an environment of support and collaboration.
• Participate actively in team meetings to discuss treatment plans and contribute insights that enhance overall patient care.

As a Licensed Clinical Social Worker at Safe Ground, you will work closely with a diverse patient demographic, including individuals facing mental health challenges, trauma, substance use issues, and chronic stress. Tailoring your approach to meet the specific cultural and emotional needs of our clients is paramount. You will empower each patient by equipping them with tools and strategies necessary for long-term recovery and resilience.

• Must possess a Master's degree in Social Work from an accredited program.
• Active licensure as a Licensed Clinical Social Worker (LCSW) in the state of Oregon is required.
• A minimum of 2-3 years of experience in a clinical setting, demonstrating proficiency in social work practice.
• Strong computer skills, including proficiency in word processing, email systems, electronic medical records, and spreadsheets, are a must.
• A proven ability to work efficiently in a fast-paced environment with the capacity to manage competing priorities.

While not required, experience in a community health clinic setting or familiarity with integrated care models is preferred. Candidates who have provided services to diverse populations or have specific training in trauma-informed care will be valued highly. Familiarity with behavioral health regulations and best practices will enhance your effectiveness in this role.
