This remote role allows you to work from the comfort of your home while still being an integral part of a dynamic clinic. You will collaborate closely with a multidisciplinary team of healthcare providers, including primary care physicians, nurses, and social workers, all dedicated to delivering holistic care to our patients. As a Licensed Clinical Professional Counselor at Crystal Lake, you will undertake a variety of responsibilities aimed at enhancing patient well-being, including:

• Conducting thorough patient intakes and comprehensive diagnostic assessments to identify mental health and psychosocial issues.
• Collaborating with primary care providers to develop individualized treatment plans tailored to each patient’s needs.
• Providing brief yet effective therapeutic interventions to patients seeking mental health support.
• Monitoring patient progress and making necessary adjustments to treatment plans based on ongoing evaluations.
• Identifying “at risk” patients and creating prevention plans to mitigate the escalation of mental health issues.
• Documenting all patient interactions, diagnostics, and treatment plans accurately and promptly in our electronic health record system.
• Facilitating seamless connections with community resources and other healthcare providers to ensure comprehensive care.
• Offering education and self-management strategies to empower patients in their recovery journey.
• Participating actively in team meetings and case discussions to contribute to overall clinic policies and treatment approaches.

Our clinic serves a diverse patient base, including children, families, and adults dealing with a variety of mental health challenges ranging from anxiety and depression to behavioral disorders. To be considered for this role, candidates must possess:

• A Master's Degree in Clinical Counseling or a related field.
• Active licensure as a Licensed Clinical Professional Counselor (LCPC).
• Comprehensive knowledge of mental health assessment, treatment methodologies, and DSM-IV diagnostic criteria.
• Strong interpersonal skills and the ability to establish productive relationships with patients and team members.
• Excellent documentation and organizational skills.
• Mental and physical health adequate to meet the demands of this role.

While not required, the ideal candidate will have:

• Experience working in a community health setting or Federally Qualified Health Center (FQHC).
• Proficiency in both English and Spanish, enhancing communication with our diverse clientele.
• A solid understanding of the community resources available for mental health support.
