Job description

First Assist, Surgical Services, Days, Full-time

The Surgical First Assistant (SFA) functions as an integral member of the perioperative team by providing direct assistance to the surgeon before, during, and after surgical procedures. The SFA performs duties that facilitate safe, efficient, and effective surgical interventions while maintaining the highest standards of patient care, aseptic technique, and professional practice.

Preoperative Responsibilities

  • Review the patient's medical record, surgical consent, and planned procedure.
  • Participate in the surgical time-out process.
  • Verify availability and functionality of required instruments, implants, equipment, and supplies.
  • Assist with patient positioning, skin preparation, and sterile draping.
  • Ensure adherence to infection prevention and patient safety standards.

Intraoperative Responsibilities

  • Provide exposure of the operative site through appropriate use of retractors and tissue handling.
  • Achieve hemostasis using approved techniques including electrocautery, clamping, tying, and topical hemostatic agents.
  • Handle tissues using atraumatic techniques.
  • Assist with wound closure, including suturing, stapling, and application of dressings as authorized.
  • Assist with placement and securing of drains and wound management devices.
  • Operate surgical equipment safely.
  • Anticipate surgeon needs and maintain an organized sterile field.
  • Participate in sponge, sharps, and instrument counts.
  • Maintain sterile technique throughout the procedure.
  • Communicate effectively with the surgical team.

Postoperative Responsibilities

  • Apply sterile dressings.
  • Assist with patient transfer from the operating table.
  • Ensure proper handling and labeling of surgical specimens.
  • Assist with room turnover and preparation for subsequent procedures.
  • Complete required documentation accurately and timely.

Additional Responsibilities

  • Participate in quality improvement initiatives.
  • Maintain required certifications and continuing education.
  • Assist with orientation and education of new perioperative staff.
  • Serve as a resource for surgical staff.
  • Support compliance with Joint Commission, AORN, OSHA, and hospital policies.

Qualifications

  • Graduate of an accredited Surgical First Assistant program or accredited Surgical Technology/Registered Nurse program with recognized first assistant training.
  • Operating room experience preferred.
  • Current CSFA, CFA, or RNFA (as applicable).
  • Current BLS. ACLS and PALS
  • Knowledge of surgical anatomy, instrumentation, and aseptic technique.
  • Ability to anticipate surgeon needs.
  • Strong communication, teamwork, and critical-thinking skills.
  • Commitment to patient safety and quality outcomes.

Physical Requirements

  • Stand for prolonged periods.
  • Lift up to 50 pounds with assistance.
  • Manual dexterity and visual acuity for surgical procedures.
  • Ability to wear required PPE for extended periods.

Working Conditions

  • Hospital operating room environment.
  • Exposure to blood, body fluids, infectious diseases, radiation, lasers, surgical smoke, and chemical sterilant.
  • Participation in call rotation, weekends, holidays, and emergency procedures as required.
