Job description

$2500 Sign-On Bonus (per diem; requires meeting minimum hours per agreement)
 Schedule: Per Diem | Flexible Scheduling| Three (3) shifts per calendar month | Regularly covering weekend shifts is expected | Two (2) holidays per year.
Department: Kresge Health & Rehab Center | Memory Care, Assisted Living, and Independent Living | Chelsea Retirement Community | Chelsea, MI
What You'll Do:
Join a 5-Star retirement community on a beautiful 58-acre campus with a rich history of excellence in senior care and a truly supportive team culture. The Speech Language Pathologist diagnoses and treats individuals with speech, language, voice, and fluency impairments as part of in-house rehab services in SNF, Assisted Living, Memory Care, and Independent Living.
  • Performs hearing and/or speech and language examinations. 
  • Evaluates hearing and /or speech and language test results. 
  • Evaluates barium swallow, other test results, and overall medical background.
  • Diagnoses patients and formulates treatment plans.
  • Educate patients and family members about the various communication methods to cope with impairment. 
  • Writes reports and maintains client information.
  • Performs administrative responsibilities: coordinates paperwork, writes lesson plans, schedules case management activities, and performs record-keeping for administrative, billing and caseload purposes. 
  • Develops individual and/or group activities and/or programs.
  • Participates in the planning and implementing of individualized educational planning (IEP) meetings, in-service meetings, or intervention assistance team meetings.

  • Graduate of an accredited Therapy program
  • Current Speech Therapist state license
  • Excellent verbal and written communication
  • Position requires the ability to lift and move objects, weighing up to 35 pounds without assistance  
  • 2 years of related experience or equivalent combination of education and experience, preferred
