Key Responsibilities

  • Conduct speech-language evaluations and develop individualized treatment plans
  • Provide skilled SLP interventions targeting dysphagia, aphasia, cognitive-communication, and voice disorders
  • Perform and interpret modified diet/texture assessments and recommend appropriate oral intake levels
  • Document evaluations, progress notes, and discharge summaries per facility and regulatory standards
  • Participate in interdisciplinary care conferences and family education
  • Maintain productivity standards and billing compliance

Requirements

  • Active state SLP license (or eligibility for endorsement/compact licensure)
  • Certificate of Clinical Competence (CCC-SLP) preferred
  • CPR/BLS certification current
  • Minimum 1–2 years of SLP experience in SNF/LTC or post-acute setting preferred
  • Experience with dysphagia management and modified diet/texture recommendations
  • Strong documentation skills and productivity management
