• Conduct comprehensive psychosocial assessments through detailed interviews, behavioral observations, and medical record reviews to establish a holistic understanding of patients’ emotional and social circumstances.
• Offer individual and group counseling to patients and their families, focusing on support systems, coping strategies, and adjustment to life changes post-rehabilitation.
• Serve as an integral part of the rehabilitation team by developing personalized treatment plans that address psychological, social, and emotional factors affecting patient recovery.
• Facilitate access to community resources—such as vocational services, housing support, and specialized support groups—to assist patients in transitioning back into their communities.
• Guide patients and families in exploring post-rehabilitation services, ensuring they are informed and prepared for long-term recovery.
• Engage in effective discharge planning to ensure smooth transitions from outpatient rehabilitation to home or alternate care environments, maintaining patient continuity of care.

You will work primarily with individuals recovering from significant health challenges, including neurological disorders, spinal cord injuries, stroke recovery, and orthopedic rehabilitation. As such, you will be instrumental in addressing the emotional, social, and practical needs that are critical for successful rehabilitation and community reintegration.

• A Master’s degree in Social Work from an accredited program is mandatory for this role.
• Active and valid state licensure as a Licensed Clinical Social Worker (LCSW) is required.
• Demonstrated knowledge of psychosocial factors related to adjustment issues, grief, and coping strategies is essential for providing quality care.
• Strong interpersonal and communication skills are necessary to establish rapport with patients and collaborate effectively with the healthcare team.
