Job description

Seeking an experienced Physician Assistant to join our Skilled Nursing Facility Service Line in Rutland, Vermont!
Details:

  • Provide primary healthcare to our geriatric population residing in Skilled Nursing Facilities in the greater Rutland Area
  • Our SNF Nurse Practitioners perform selective medical services through a collaborative agreement with a supervising MD/DO
  • Must enjoy working with a dedicated team of highly motivated professionals who are committed to providing an integrated, community-based approach to care
  • 5+ years Physician Assistant experience in an office setting preferred


Essential functions include though are not limited to
:

  • Obtains complete medical history and physical examination on patients
  • Reviews medical records
  • Interprets and integrates data to determine appropriate diagnostic and therapeutic procedures
  • Establishes presumptive diagnosis
  • Orders appropriate laboratory and diagnostic procedures
  • Develops a therapeutic plan for patients
  • Provides orders for drugs and controlled substances
  • Serves as the resource โ€œgo toโ€ Provider for designated SNF
  • Collaborates and coordinates care with SNF Care Coordinators
  • Works collaboratively with other community partners/Providers to optimize patient care
  • Assists in the development and monitoring of Quality Improvement for SNF residents
  • Participates in โ€œon call โ€œcoverage for SNF Service Line
  • Provides ongoing continuity of care
