Communication will primarily occur through virtual platforms, ensuring that you remain connected with your clients and the interdisciplinary team at our Hospice Center. • Develop, implement, and manage personalized support plans for clients facing psychological and emotional challenges.
• Conduct thorough assessments with service users to determine their immediate needs as well as their strengths and areas for growth.
• Set measurable and achievable goals tailored to each individual case, monitoring progress meticulously.
• Provide ongoing support and counseling during client sessions, offering informed recommendations for their care.
• Keep accurate and confidential records of client interactions and progress reports, adhering to all legal and ethical guidelines.
• Collaborate with multidisciplinary teams, attending case conferences and contributing valuable insights regarding client care.
• Facilitate referrals to external community services and resources as needed to support clients’ recovery journeys.
• Act as a key-worker, maintaining strong relationships with clients to ensure they receive tailored and coherent care.

Your primary focus will be on clients located within the Hospice Center setting, where you will engage with patients who are facing critical end-of-life transitions. This role requires sensitivity and a solid understanding of the unique challenges faced by individuals nearing the end of life, including grief, loss, and existential concerns. You will work to empower clients and their families during this vulnerable time through thoughtful and individualized care plans.

• A Bachelor’s Degree in Social Work (BSW), Psychology, Sociology, or a related field is required; candidates with a Master’s degree are preferred.
• Licensed Clinical Social Worker designation is essential to practice.
• Proven experience in clinical social work, with a focus on hospice or palliative care being highly advantageous.
• Demonstrated proficiency in social work theories and methodologies relevant to mental health.
• Strong empathetic skills and a deep understanding of the psychosocial dynamics of illness.

• Experience working in remote or telehealth settings is highly desirable.
• Familiarity with electronic health record (EHR) systems and remote communication tools.
• A background in working with a diverse client population, exhibiting cultural competency and sensitivity.
• Skills in teamwork and collaboration within interdisciplinary healthcare environments.
