Primary Work Location: KCC

 The Kelly Cullen Community is located in the Tenderloin neighborhood of San Francisco and is home to 174 individuals with extremely low incomes, including those that experienced homelessness and are currently living with health challenges. On-site social services are run by the Tenderloin Neighborhood Development Corporation, clinical services are provided by Cardea Health. 

Full-time, part-time, and per diem positions available

Schedule: Monday - Friday PM shifts from 12:00 PM–8:30 PM, with alternating weekend shifts from 9:00 AM–5:30 PM.

Job Summary:  Cardea Health Licensed Vocational Nurses assist clients with their medical care needs, implement nursing care plans and maintain medical records. LVNs at the KCC will administer nursing care to ill, injured, convalescent or disabled clients. LVNs may advise clients on health maintenance and disease prevention or provide case management services.    

Job Duties:

  • Collaborate with housing staff to facilitate resident enrollment in medical benefits, connection with medical home, and other social services 

  • Collaborate with resident’s Primary Care and other professional disciplines to ensure effective care delivery and achievement of desired outcomes 

  • Ensure residents are provided with appropriate information to make informed choices related to treatment and that care reflects resident preferences 

  • Maintain communication with Homekey partner pharmacies, facilitate safe medication use on site in accordance with individual resident needs 

  • Ensure adherence to infection control standards on site among staff and residents 

  • In collaboration with housing agency, ensure daily wellness of residents 

  • Identify and respond to immunization needs of residents 

  • Plan and execute population health interventions among residents and staff that contribute to facility safety and health 

  • Ensure that clinical documentation is current, timely, and accurate 

  • Maintain confidentially in accordance with the Health Insurance Portability and Accountability Act (HIPAA) 

  • Utilize and contribute to county data platforms such as the Community Health Record, Salesforce platform, and others 

  • Recognize and respond to medical emergencies on site, follow up with MD communication or 911 activation 

  • Facilitate post-hospital transition plan of care for residents discharged from Emergency Department or inpatient hospital stays 

  • Plan and execute interventions to reduce number of ambulatory-sensitive Emergency Department visits 

  • Practice harm reduction principles on site, including interventions that minimize harmful effects of substance use while treating all residents with dignity and respect 

  • Other duties and responsibilities as required 

Minimum Required Skills & Qualifications:  

  • Graduate from an accredited vocational nursing program

  • Hold current license as Licensed Vocational Nurse through Board of Vocational Nursing and Psychiatric Technicians 

  • Current Basic Life Support Certification 

Preferred Qualifications: 

  • Experience with homeless or incarcerated populations is highly desirable

Physical Requirements:  

  • Occasional sitting or stationary work 

  • Frequent standing, walking, kneeling, stooping, and ascending stairs 

  • Occasional lifting and carrying up to 20 lbs 

  • Frequent pushing and pulling 

  • Frequent reaching high, low, and level 

Work Environment:  

  • In-person onsite at a supportive housing facility. Constant work around and/or with people OR Interaction with patients, clients, and external customers
